CONTRACT AMENDMENT NUMBER 1
SCOPE OF SERVICES
This Project consists of design and construction related services to develop recharge facilities
for treated effluent from the Arrowhead Ranch Water Reclamation Facility. Phase 1,
evaluation and identification of recharge alternatives on the 10-acre site near 79th Avenue and
Utopia, has been completed. The Scope of Services for this contract amendment are for Phase
2 of this project which includes: permitting, preparation of bid documents, and bid phase
services.
A. Phase 2 - Preparation of Bid Documents
1. Site Survey. The Engineer will prepare topographic maps of the 10-acre site.
Field surveys will include setting the panels for the aerial photography and
locating utilities that may impact final design. The scope of these services does
not include a boundary survey of the site(s) or completion of ALTA survey
plat(s).
2. Construction Plans and Specifications. The Engineer, in conjunction with sub
consultants, will prepare plans and specifications for the recommended
improvements in the Phase 1 Study. Plans and specifications will be prepared
using Glendale's standard format and MAG standards as modified by the City.
Plans will be submitted for the City's review at approximately 60%, 90% and
100% for review and comment. The design will include the following principal
components and other incidental items required for a complete and operational
system:
-2 Vadose Zone Recharge Wells
-2 Aquifer Storage and Recovery Wells
-On-Site Lake With Liner and Aeration
-Irrigation Pump Station for Adjacent City Park
-On-Site Reclaimed Water Piping
-New Electrical Service Entrance and On-Site Distribution
-Instrumentation and Control System with Telemetry to the ARWRF
All construction plans and specifications will be prepared by Wilson & Company
with the exception of the well drilling specifications and the electrical,
instrumentation and control plans and specifications. The well drilling
specifications will be prepared by our hydrogeologic sub consultant, Southwest
Ground-water Consultants, Inc. (SGC). The electrical, instrumentation and

control plans and specifications will be prepared by our electrical subsconsultant
DL engineering & controls, Inc (DLEC). The attached SGC and DLEC scope of
services further define the roles of the sub consultants in the preparation of the
Construction Plans and Specifications.
3. Cost Estimate. The Engineer will prepare budget cost estimates at the 60%,
90%, and 100% completion stages.
4. Permit Applications. The Engineer, in conjunction with sub consultants, will
prepare applications for and assist the City in obtaining the ADWR Facility and
Storage Permit, the ADEQ Aquifer Protection Permit, the Maricopa County
Approval to Construct, and the ADWR Recovery Well Permit.
Wilson & Company will prepare and submit the Maricopa County Approval to
Construct application package. Our sub consultant, Goldman, Toy &
Associates, Inc. (GTA), will be the primary consultant responsible for preparing
the permit application packages for and coordinating with ADWR and ADEQ.
Our sub consultant, Southwest Ground-water Consultants, Inc. (SGC), will be
responsible for preparing the hydrogeologic portions of the ADWR and ADEQ
permits. Wilson & Company will be responsible for reviewing and coordinating
all ADWR and ADEQ permit applications. The attached SGC and GTA scope of
services further define the roles of the sub consultants in the permitting
activities.
The Engineer will assist in completing the City's internal building/plan reviews.
This scope of services does not include preparing a Section 404 Permit or
conducting any cultural, historic and/or endangered species surveys. All permit
and review fees will be paid by the City.
This task also includes the installation of a permanent sampling pump in the
existing MW-1 monitoring well that was installed in Phase 1 of this project.
This will allow for the collection of water quality samples, as recommended by
ADEQ, from this well to define background ambient groundwater quality in
support of the permit application process prior to beginning recharge operations.
The installation of this pump will be arranged for and coordinated by our sub
consultant, SWG. The attached SWG memorandum further defines the
equipment to be installed in MW-1. The City will collect the required samples
and pay all costs associated with performing the laboratory analysis.
5. Public Meetings. The Engineer will assist the City in conducting two public
meetings during the design phase. The Engineer's activities will be limited to
preparing exhibits showing the scope of the planned construction activities and
significant project features as well as providing staff to attend both meetings.
The City will be responsible for compiling the mailing lists, notifying the public,
arranging the meeting location and providing refreshments.

6. Monthly Meetings. The Engineer will meet with City staff once each month to
review progress of the project, identify outstanding issues and assign tasks
necessary to complete the plans and specifications in a timely manner.
B. Phase 3 - Bid Services
1. Prebid Conference. The Engineer will attend a Prebid Conference to review the
Bid Documents with prospective bidders. The Engineer will prepare meeting
minutes to document responses to bidders questions.
2. Materials and Equipment Preapproval. The Engineer will review bidders requests
for materials and equipment preapproval and issue an addendum detailing all
preapproved alternatives.
3. Interpretations. The Engineer will be available to answer questions from
prospective bidders by telephone.
4. Addenda. The Engineer will issue such addenda as may be required to clarify
the intent of the bid documents.
5. Bid Opening. The Engineer will attend the bid opening, assist in reviewing the
qualifications of the low bidder, and notify the City of any complaints filed with
the Registrar of Contractors against the low bidder.
6. Services Provided by the City. The City will print and distribute Bid Documents
to prospective bidders, maintain the plan holders list, mail such addenda to plan
holders as may be required to clarify the Bid Documents and prepare the Bid
Tabulation.

SCOPE OF WORK -

2. Construction Plans and Specifications
SOC will prepare plans and specifications for four (4) test wells. Specifically, SGC will
develop conceptual designs for two (2) vadose zone and two (2) aquifer storage and
recovery (ASR) wells. Following approval of these designs by Wilson & Co. and the
City of Glendale, SGC will prepare specifications for the drilling, construction and
testing of these wells.
3. Cost Estimate
SOC will prepare cost estimates for the drilling, construction and testing of the four (4)
recharge wells.

4. Permit Applications
SGC will provide necessary support to Wilson & Co. and the City of Glendale in the
preparation and acquisition of permits for a Pilot Underground storage and Recovery
Project. Specifically, SGC will:

• Prepare a hydrologic report for submission with the Underground Storage
Facility Permit
• Establish the area of impact (AOI)
• Prepare hydrogeologic and water quality sections of the ADEQ Aquifer
Protection Permit (APP) application (Characterization, Impact, Monitoring
and Contingency)
• Prepare hydrogeologic portion of recovery well permits, as appropriate
To the extent possible, this task will utilize data and analyses compiled and completed hi
Phase I. In addition, SGC anticipates the development of a computer model of the area
for conduct of the mounding analysis and calculation of the AOI.

Introduction
The Arrowhead Ranch Effluent Disposal:System is a City of Glendale project
which will dispose of excess effluent in the Arrowhead Ranch area. The project is
being carried out in several phases. Phase 1, which was recently completed, estimated
that the City's 10-acre site could recharge approximately 1,000 gpm. Phase 2 of the
project includes the engineering and permit applications for a pilot recharge/recovery
project to test the sites recharge capabilities. The Phase 2 work includes the Prepar-ation
of Bid Documents and I)esigns for 2 Vadose Zone Recharge Wells and 2 Aquifer
Storage and Recovery Wells; Associated Permits for ADEQ, ADWR, and Maricopa
Count; Public meetings; and Bid Services.
GTA will be the lead consultant to prepare applications for the ADWR Pernits and
ADEQ Aquifer Protection Permit. SGC will be responsible for preparing the hydrologic
data and carrying out the computer modeling of the aquifer systems response to the
recharge project. Wilson and Co. will also provide technical support as it relates lo the
detailed desigm and public involvement. GTA will provide technical comments on the
Wilson and Co. designs on the ASR wells.
Wilson and Co.,will provide the overall project coordinaticn; carry out the detailed.
designs of the vadose zone and ASR. wells, public involvement, provide the
coordination with City, and provide the bid services.
GTA tasks are as follows:
Task 2 - Construction Plans and Specifications Wilson and Co. will be responsible for
the design and production of the plans and specifications of the vadose zone and ASP.

designs. We anticipate GTA will review the 10%, 50% and 90% ASR well detailed
designs.
Task 5 : Permit Applications GTA will be the lead consultant and will prepare the
applications for the ADWR Pilot Recharge Projects, the ADWR well recover permit,
and the ADEQ Aquifer Protection Permit (APP). GTA's work will include preparing
the documents necessary for pre application meetings withe ADEQ and ADWR and
attending the pre application meetings. GTA will prepare the permit draft application
packages which include, maps, project descriptions, description of the recharge
methods, hydrogeolgic information, land ownership information, recharge quantities,
site layout, flow measurement devices, and other information required by the agencies
GTA will coordinate with SGC to present the hydrogeolgic information and the
results of the aquifer simulation computer model. SGC will be responsible for all
technical information related to the aquifer and computer model. Wilson and Co., will
provide sketches for the site layout which show the recharge project.

Responsibilities of Others •
1. CLIENT shall furnish CAD files of P84ID drawings showing process piping and structures in
schematic form. DLEC will then add the instrumentation and control signal schematics to
those drawings.
2. CLIENT shall furnish CAD files of the project plans to DLEC in AutoCAD v14.
3. CLIENT shall furnish the electrical power and instrumentation requirements for the
equipment to be specified on this project, and connected to the power distribution system:
4. CLIENT shall provide descriptions of process control requirements, and identify area
classifications per NFPA 820, and other related codes.
. Deliverables
I. One set of review documents for each submittal phase will be provided in reproducible form.
CLIENT will be responsible for duplicating documents for submittal to the Client.
2. Upon completion of the design, we will provide one set of final sealed documents bearing the
signature of a registered electrical engineer in the State of Arizona. Please specify in advance
if the plans are to be plotted on vellum or mylar.
Exclusions and Exceptions
1: DLEC will not obtain any permits.
2. DLEC will not be responsible for reproduction of submittal documents for the
Client/Owner's review.
3. On P&Ill drawings, DLEC will not prepare schematic design of process piping and
structures.
Billing Rates:
Our Lump Sum fees for the aforementioned work will be for services provided as follows:
1. Electrical Design Services: Twenty Six Thousand One Hundred Thirty Six Dollars
($26,136.00)
2. Bid Services: One Thousand Seven Hundred Forty Seven Dollars ($1,747.00)
3. Reimbursable expenses: Three Hundred Sixty Dollars ($360.00)

Please refer to the attached Proposal Worksheet(s) for our hourly rates, expenses, and labor-hour
breakdown. Any additional services requested by CLIENT that are not included herein will be
charged at our Standard Hourly Rates as shown on the attached Proposal Worksheet(s). This
proposal will be valid for the next ninety(90) days.
We will bill you on a monthly basis for these services. Payment terms are Net 30 days. If you
have any question, please contact us at your earliest convenience.
