Notice: Undersigned Borrower/Trastor is participating in federal programs pursuant
to 42 U.S.C. $ J30l et seq. and/or in the Horne Investment Partnership Act under 42
U.S. C. S 12701 et seq. In accordance with applicablefederal law, the City of
Glendale acts as the "pass-tltrough agency" for tlrc distibution of tlusefederalfunds.
This Modilication to Promissory Note and/or Deed of Tntst is stntctured n be compli-
ant with the above-referenced federal laws, regulations, and guidelines.
THIS AGREEMENT, made this !&b day of Aueust 2015 by and between City of Glendde, as
knder/Beneficiary and Paficia Rrmirez, as Bottowers/Trustors, under that certain homissory Note and M
of Trust in the original principal sum of Nineteen Thourend Dollrrs ($191000.00) &ted Mey 25, 2016. The
Deed of Trust securing the Promissory Note was recorded in the Office of the County Recorder of Maricopa
County, Sate of Arizona on May 31, 2016 at Instnrmeirt No. 20160372474, covering the following described
real property:
THE SOUTH HALF OF THE EAST HALF OF THE WEST TWO.TENTHS OF THE SOUTE HALF
oF LOT 3 IN BLOCK E, ORCHARD ADDnON TO GLENDALE, ACCORDING TO BOOK 3 OF
MAPS, PAGE 30, RECORDS OF MARTCOPA COI'NTY, ARIZONA-
