RESIDENTIAL REHABILITATION PROGRAM
CONTRACTOR'S REQUIREMENTS
1. Must be a licensed General Contractor in good standing with the State of Arizona
Registrar of Contractors, and submit proof of the following coverage:
A. Manufacturers & Contractors Liability $1,000,000 per person/$1,000,000
per occurrence
B. Property Damage $60,000/$120,000. (A certificate naming the owner
must be submitted for each job.)
C. Workman's Compensation Insurance for all subcontractors engaged in work
on the Contract Premises, in accurdance with local and State laws governing
same.
2. If Contractor is incorporated, must submit proof of incorporation and a list of the
approved signers of any agreement the corporation might enter into.
3. Must be willing to participate in the federal Minority and Women's Business Enterprise
Policy (MBE/WBE) whenever feasible, if subcoZoe . 'zed.

Remove existing deteriorated garage door and demo off -site. Install new new metal 4-5 section,
overhead garage door complete with stops and all bracing required for installation. installation to
include all hardware. Door shall have exterior handle/locking mechanism device. Use medium grade
door equal to ANOZIRA GARAGE DOOR CORP. or equal in quality. Door shall have no glass.
Contractor will not make substitutions without written authorization from Revitalization Coordinator.
Paint all exposed new wood due to installation. New door shall be painted color of residence.
Homeowner may change color prior to painting only. -

Remove existing windows and demo off -site. Install new aluminium, slider type, dual glaze windows
of the same sizes as existing, INCLUDING BUG SCREENS. Windows will be (BETTER BUILT or
EQUAL). Install new SHADE SCREENS on all windows. Owner shall have choice of color, if other than
charcoal available. Bedroom windows WILL meet all Ingress/Egress building code requirements
unless otherwise stated. INSTALLATION: SIDING: cut straight cuts around windows to remove. Any
exterior siding finish cut for installation must be "toothed" back in with siding so joints are
staggered. STUCCO: Stucco finishes to be blended to match adjacent wall area's. MASONARY: DO
NOT WIDEN WINDOW OPENING. Drop sill, "toothing" in any fill block and properly install new sill
block. Wood will not be allowed as a filler. Properly caulk and finish the cut surface of the block to
match adjacent wall area's. Contractor will measure and verify all sizes before ordering windows.
Repair all interior and exterior finishes affected by installation and paint to match adjacent wall area's
to match as close as possible. Remove all debris as soon as possible.
NOTE: Homeowner is responsible for removal of all pre-existing security devices (contacts)
attached to windows and doors and for re -installation of devices.

installed plumb with minimum " clearance between jamb & trimmer for adjustment. Door will be

Demo and remove from site existing wall & base cabinets. Provide and install new prefinished wall &
base cabinets. New cabinet installation shall follow old cabinet layout as close as possible, unless
otherwise specified. cabinets shall be equal to or exceed MERILLAT, AMERICANA or TRIPAC MODELS
and approved by the NATIONAL KITCHEN AND BATH ASSN. All cabinets shall be labeled with the
manufacturer's name and shall be FHA approved. Shelving will be full sized in upper and lower
cabinets. Shelves will be adjustable. Face frame to be solid oak, with insert flush panel having
mortise and tenon joints. LAZY SUSAN STYLE BASE CORNER CABINETS WILL BE INSTALLED IF
POSSIBLE. Cabinet color to be medium oak tone stain. All cabinet trim material will be actual oak
(wood), matching as close as possible. Homeowner accepts that due tct installatioM of new cabinets,

Remove existing countertop and demo off -site. Install new countertop using post `6' rrrred plastic
laminate equal to FORMICA or WILSON ART. Countertop shall be 1/16" thick minimum, including 4"
blacksplash at all rear walls and end walls. Countertop will be scribed to fit all walls. Caulk'any
remaining cracks with vinyl caulking. Top must be set level. End caps must be installed in matching
plastic and neatly trimmed.
Secure top to cabinets @ 2" OC front and rear with screws. Chips in plastic finish are not accepable.
Owner will have choice of color. High gloss type tops will not be offered.

Remove existing carpet and haul off site. Install new FHA approved 29 Oz., 100% nylon, continous
filament carpet, over 1/2" rebonded pad. Pad shall meet or exceed FHA minimum requirement for
the carpet installed. New carpet shall have a 10 year warranty and a 5 year stain resistant warranty.
Installation shall include tackless strips. Use top metal on terminations. Power stretch carpet to
eliminate puckers, scallops and ripples. Pile direction for all carpet installed that is adjoining. Use
maximum 2 pieces in closets. Carpet shall be WORLD CARPET "Shangri-La", QUEEN CARPET
"Westwood" or COLUMBUS "Adorable". One color thru out residence. Homeowner shall have choice
