Box 22035 Tempe,AZ 85285 1 Re: Vehicle impact damage assessment Lilly Irwin Residence 5705 N. 45th Drive Glendale,AZ 85301 II Claim#03H196746 il 1 Dear Ms. Iverson: As requested, I met with Ms. Lilly Irwin at the above referenced claim on 1 I September 30, 2005. The purpose of this meeting was to examine the damage to this residence from a recent vehicle impact and provide recommendations for appropriate structural repairs. 1 OBSERVATIONS: The following photographs were taken during this inspection: INO: COMMENTS: - 1. Front elevation of residence. The vehicle was southbound on 45th Drive and I. impacted and damaged the garage,planter wall and front yard fence wall. .. 2. Close-up of the board-up of the garage door wall of the residence. 3. Note that the gable end sheeting and trim around the garage door was damaged. t r Ms. Gwen Iverson October 3,2005 47- Close-up of damage-to the garage gable end sheeting and trim at the south end of the garage door. • 5 - There was a masonry pier at the north end of the garage door opening that was not damaged in the impact. 6. The evidence indicates that the original pier at the south end of the garage door was constructed of frame, not masonry like the north end shown in photo#5. 7. Close-up of the junction's between the south'masonry wall of the garage and the location of the original garage door pier. Note that the original wood siding its still. attached to'the`block'indicating that:this pier.was originally frame and not block. - 8 The bottom wood plate for the south garage door pier is still in place. 9. The south wall of the garage was severely damaged and is bowed outward. 10. Close-up of damage to the south wall of the garage shown in photo#9. 11. The bottom course of block has been shifted off of the concrete stem wall at the south wall of the garage. -12. There is a sag in the roof over the entry. This roof was framed by sistering roof joists onto the tails of the garage roof framing and the sag in this roof appears to pre-exist the vehicle impact damage. 13. The stair step cracking at the south wall of the garage extends the full length of the wall. 17. Close-up of stair step cracking at the east end of the south wall of the garage. 18. The south wall of the garage has been displaced off the concrete stem wall at the west end. 19. There is minor stair step cracking and some loose paint that appears fresh at the south end of the front wall of the house. 22. Vehicle impact damage at the front yard fence and planter. —23. No damage or evidence of movement was noted at the interior of this room. 25. Interior view of the board up of the west wall of the garage. 26. There is minor stair step crackingat the west end of the north wallof the house. This damage is too small to tuck point and can be repaired by painting. CONCLUSIONS AND RECOIVIlVIENDATIONS: • The following vehicle impact damage assessment and recommended repairs is submitted: 1. Remove and replace the south pier (2'-0" x 7'-0") of the garage with. new -- twall studs and siding. The visual evidence indicates that this pier was originally wood frame and not masonry. 2. Remove and replace the garage door and trim. �- 3. Remove and replace or repair the wall studs, header and siding at the gable end wall above the garage door.- 4. Remove and replace the entire south masonry wall of the garage (1.4'-0" x 8'-4")with new 8"masonry as per the attached details and general structural notes. 3 i . VMs. Gwen Iverson i October 3, 2005 . L 1 5. Remove and replace the damaged portions of the front fence wall and P g planter. 6. There are hairline cracks at the north wall of the garage and at the west wall 1 of the main house. These cracks are a cosmetic concern and not a structural stability or safety concern. The cracking in the paint appears fresh and I recommend that these walls be repainted as a part of this vehicle impact damage repairs for the house. Some minor tuck pointing or caulking can be done at the south end of the west wall,however the crack at the north wall of 111 the garage are too small to tuck point. 1 7. Re-hang and adjust the entry security gate after replacement of the south masonry wall of the garage so that it closes and latches smoothly.
