1. CALL TO ORDER THE LOCAL POLICE BOARD
Chair Charles Erickson called the meeting to order at 9:35 a.m.

4. APPROVAL OF THE MINUTES
The Board voted to approve the Public Safety Personnel Retirement System - Local Police
Board Minutes of October 28, 2020

6. NEW BUSINESS
A. Board Secretary Jim Brown read the pertinent information and the Board reviewed the
Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.) submitted
by Charles J. Bayer, Police Sergeant. Charles J. Bayer has 30.084 years of credited
service. He has elected to participate in the D.R.O.P. on January 8, 2021 and must
terminate employment with the City of Glendale Police Department on or before January
31, 2026.
The Board voted to approve the application.

B. Mr. Brown read the pertinent information and the Board reviewed the Application for

Michael Chambers, Police Officer. Jason Michael Chambers has 22.13 years of credited
service. He has elected to participate in the D.R.O.P. on January 1, 2021 and must
terminate employment with the City of Glendale Police Department on or before January
31, 2026.
The Board voted to approve the application.

C. Mr. Brown read the pertinent information and the Board reviewed the Application for

Garrett, Police Officer. Kevin Garrett has 20.022 years of credited service. He has elected
to participate in the D.R.O.P. on December 4, 2020 and must terminate employment with
the City of Glendale Police Department on or before December 31, 2025.
The Board voted to approve the application.

D. Mr. Brown read the pertinent information and the Board reviewed the Application for
Deferred Annuity from the Public Safety Personnel Retirement System (PSPRS) submitted
by Richard Chadwick, former Police Officer, on December 2, 2020. He terminated his
employment with the City of Glendale Police Department on November 20, 2020. At the
time of his termination, he had 12.394 years of credited service. He became eligible for this
benefit on February 15, 2019.
The Board voted to approve the application.

6. EXECUTIVE SESSION
Upon a public majority vote of a quorum of the Local Police Board, the Board may hold an
executive session, which will not be open to the public, regarding any item listed on the
agenda but only for the following purposes:
(i) discussion or consideration of records exempt by law from public inspection (A.R.S. §
38-431.03(A)(2));
(ii) discussion or consultation for legal advice with the Board's attorneys (A.R.S. §
38-431.03(A)(3));
The Board voted to adjourn open session and go into Executive Session in order to review
confidential medical records and to -receive legal advice from the Board's attorney at 9:45
a. m.
Prior to any discussion in Executive Session, Police Officers Jarod Behrens and Randall
Campbell exited the conference room.

A. During Executive Session the Board reviewed Medical Evaluations for three (3) New Police
Personnel:
• Victor J. Becerra
• Jonathan Lopez
• Jadan Smith
B. During Executive Session the Board reviewed and discussed the Application for Accidental
Disability Retirement from the Public Safety Personnel Retirement System (PSPRS)
submitted by Police Officer Jarod Behrens, along with the Independent Medical
Examination report from the Medical Board and additional documents.
C. During Executive Session the Board reviewed and discussed the Application for Accidental
Disability Retirement from the Public Safety Personnel Retirement System (PSPRS) along
with medical records, submitted by Police Officer Randall Campbell.
7. ADJOURN EXECUTIVE SESSION AND RECONVENE IN OPEN SESSION
The Board reconvened in open session at 10:20 a.m.
Officers Behrens and Campbell returned to the conference room.
8. CONSIDERATION AND POSSIBLE VOTE ON EXECUTIVE SESSION ITEMS
A. Mr. Brown confirmed that the following new Police Personnel meet the standards of the
Glendale Police Department and the Arizona Peace Officer Standards and Training Board.
The Board voted to accept three (3) New Police Personnel into the Public Safety Personnel
Retirement System (PSPRS) with any pre-existing conditions noted in their confidential
medical records:
• Victor J. Becerra
• Jonathan Lopez
• Jadan Smith

B. Chair Erickson asked Mr. Behrens if he would like to address the Board. Mr. Behrens
replied that he did not.
Board Member Cody Carmichael motioned and the Board voted to order a second IME
(Independent Medical Evaluation) to evaluate additional dates of injury as indicated in Mr.
Behrens' letter and resolve questions about the source of mental condition that the Board's
first IME provider did not address.

C. Chair Erickson asked Mr. Campbell if he would like to address the Board. Mr. Campbell
replied that he did not.
Board Member Justin Harris motioned and the Board voted to deny the application for
Accidental Disability Retirement submitted by Randall Campbell because the applicant is
still receiving medical treatment to determine whether the mental condition is total and
permanent.

9. BOARD COMMENTS AND SUGGESTIONS
There were no Board comments or suggestions.
10. NEXT MEETING
The next regular meeting of the Public Safety Personnel Retirement System Local Police
Board will be held on January 27, 2021 at 9:30 a.m., in the HR Training Room, located
at 5850 W. Glendale Ave., Glendale, Arizona, 85301.
11. ADJOURN THE LOCAL POLICE BOARD MEETING
Chair Erickson adjourned the meeting at 10:25 a.m.
The Public Safety Personnel Retirement System Local Fire & Police Boards meeting minutes of
December 16, 2020 were submitted and approved this 27th day of January, 2021.
