CALL TO ORDER THE LOCAL POLICE BOARD
Board Member and Acting Chair Charles Erickson called the meeting to order at 9:31 a.m.

5. NEW BUSINESS
A. The Board reviewed an Application for Enrollment into the Deferred Retirement Option Plan
(D.R.O.P.) submitted by Anthony D. Brown, Police Sergeant. Anthony D. Brown has 28.505
years of credited service. He elected to participate in the D.R.O.P. on December 6, 2018 and
must terminate employment with the City of Glendale Police Department on or before
December 31, 2023.

B. The Board reviewed an Application for Enrollment into the Deferred Retirement Option Plan
(D.R.O.P.) submitted by David Garrett Madeya, Police Lieutenant. David Garrett Madeya has
25.372 years of credited service. He elected to participate in the D.R.O.P. on December 28,
2018 and must terminate employment with the City of Glendale Police Department on or
before December 31, 2023.

The Board reviewed an Application for Enrollment into the Deferred Retirement Option Plan
(D.R.O.P.) submitted by Eric Rouse, Police Officer. Eric Rouse has 20.003 years of credited
service. He elected to participate in the D.R.O.P. on November 18, 2018 and must terminate
employment with the City of Glendale Police Department on or before November 30, 2023.

D. The Board reviewed an Application for Retirement from the Deferred Retirement Option Plan
(D.R.O.P.) submitted by Thomas Robbins, Police Officer. Thomas Robbins has elected to
retire from the D.R.O.P. and has terminated his employment with the Glendale Police
Department on November 1, 2018. On March 28, 2018, the Local Police Board had approved
his Application for Enrollment into the D.R.O.P. and he had elected to participate on May 25,
2018. He was required to terminate his employment with the City of Glendale Police
Department on or before May 31,2023.

E. The Board reviewed the Application for Survivor's Benefits from the Public Safety Personnel
Retirement System (PSPRS) submitted by Betty J. McClendon, spouse of deceased
Glendale Police Officer, John E. McClendon. John passed away on October 26, 2018.

F. The Board acknowledged receipt of an Application for Accidental Disability Retirement from
the Public Safety Personnel Retirement System (PSPRS) submitted by Justin Penrose, Police
Officer. It was noted that medical records submitted with application were to be reviewed
during Executive Session.
6. EXECUTIVE SESSION
Upon a public majority vote of a quorum of the Local Police Board, the Board may hold an
executive session, which will not be open to the public, regarding any item listed on the
agenda but only for the following purposes:
(i) discussion or consideration of personnel matters (A.R.S. § 38-431.03(A)(1));
(ii) discussion or consideration of records exempt by law from public inspection (A.R.S. §
38-431.03(A)(2));
(iii) discussion or consultation for legal advice with the city's attorneys (A.R.S. §
38-431.03(A)(3));
A. Medical records for Police Officer Justin Penrose were reviewed by the Board in
conjunction with his Application for Accidental Disability Retirement from the Public Safety
Personnel Retirement System (PSPRS).
B. Medical Evaluations for three (3) New Police Personnel were reviewed by the Board:
Sawyer Brenae Burruel
Joseph Brandon Garcia Quinonez
Sydney Rose Sutter
7. ADJOURN EXECUTIVE SESSION AND RECONVENE IN OPEN SESSION
8. APPLICATION FOR ACCIDENTAL DISABILITY RETIREIMENT
The Board acknowledges that Police Officer Justin Penrose has filed an Application for
Accidental Disability Retirement from the Public Safety Personnel Retirement System
(PSPRS), and has directed that an Independent Medical Examination be conducted.
9. CONSIDERATION AND POSSIBLE VOTE ON EXECUTIVE SESSION ITEMS
Police Office Michele Blanco confirmed that the following new Police Personnel met the
standards of the Glendale Police Department and the Arizona Peace Officer Standards and
Training Board:

10. BOARD COMMENTS AND SUGGESTIONS
Board Member Charles Erickson mentioned that his appointment to the Local Police Board will
expire in January 2019, and that he had notified the City Clerk's office of his desire to continue
as a Board Member. Jim Brown said that he would verify with the City Clerk's office that Mr
Erickson has notified them that he would like to continue as a Board Member.
11. NEXT MEETING
The next regular meeting of the Public Safety Personnel Retirement System Local Police
Board will be held on December 19, 2018 at 9:30 a.m., in the 4th Floor Conference Room,
located at 5850 W. Glendale Ave., Glendale, Arizona, 85301.
12. ADJOURN THE LOCAL POLICE BOARD KflEETING
Board Member and Acting Chair Charles Erickson adjourned the meeting at 9:52 a.m.
The Public Safety Personnel Retirement System Local Fire & Police Boards meeting minutes of
November 21, 2018 were submitted and approved this 19th day of December, 2018.
