CALL TO ORDER THE LOCAL POLICE BOARD Board Member and Acting Chair Charles Erickson called the meeting to order at 9:33 a.m. 2. • ROLL CALL Members Present: Mayor Jerry Weiers City Manager Kevin Phelps Board Member Charles Erickson Board Member Cody Carmichael Board Member Justin Harris Members Absent: None Staff Present: Jim Brown, Director of Human Resources and Risk Mgmt. Nancy Mangone, Assistant City Attorney Charlotte Beadles, HR Specialist, Benefits Lisa Chill, HR Management Assistant Michele Blanco, Police Officer Other Attendees: Attorney Carrie O'Brien 3. CITIZEN COMMENTS No citizens were present. 4. APPROVAL OF THE MINUTES Public Safety Personnel Retirement System - Local Police Board Minutes of August 22, 2018 Motioned by Board Member Justin Harris, seconded by Board Member Kevin Phelps AYE: Mayor Jerry Weiers Board Member Cody Carmichael Board Member Charles Erickson Board Member Justin Harris Board Member Kevin Phelps Passed 5. NEW BUSINESS A. Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.) - Frank Michael Crawford, Police Officer. Frank Michael Crawford has 21.062 years of credited service. He elected to participate in the D.R.O.P. on September 30, 2018 and must terminate employment with the City of Glendale Police Department on or before September 30, 2023. Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.) - Elizabeth Ann Diaz, Police Officer. Elizabeth Ann Diaz has 20.063 years of credited service. She elected to participate in the D.R.O.P. on September 1, 2018 and must terminate employment with the City of Glendale Police Department on or before September 30, 2023. An Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.) - Raymond LeVander, Assistant Police Chief has been received. The Human Resources Department had noted that he has 24.241 years of credited service on the benefits calculation form. Officer Cody Carmichael stated that he had a conversation with Raymond LeVander who thought the years of credited service should be closer to 27 years or more, including previous service. HR Benefits Specialist Charlotte Beadles explained that all numbers on the calulation sheet submitted to the Board with the application are estimates and that the years.of service and estimated monthly benefit amounts will be verified before submitting the final application to PSPRS. The Board decided to vote to approve the Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.) contingent upon all corrections being made by the Board Secretary before the final application is submitted to PSPRS. An Application for Accidental Disability Retirement from the Public Safety Personnel Retirement System (PSPRS) from Police Officer Wesley Zygmont has been received. Medical records submitted with application will be reviewed during Executive Session. 6. EXECUTIVE SESSION Upon a public majority vote of a quorum of the Local Police Board, the Board may hold an executive session, which will not be open to the public, regarding any item listed on the agenda but only for the following purposes: (i) discussion or consideration of personnel matters (A.R.S. § 38-431.03(A)(1)); (ii) discussion or consideration of records exempt by law from public inspection (A.R.S. § 38-431.03(A)(2)); (iii) discussion or consultation for legal advice with the city's attorneys (A.R.S. § 38-431.03(A)(3)); Motioned by Board Member Cody Carmichael, seconded by Board Member Justin Harris AYE: Mayor Jerry Weiers Board Member Cody Carmichael Board Member Charles Erickson Board Member Justin Harris Board Member Kevin Phelps Passed A. Medical records for Police Officer Wesley Zygmont were reviewed by the Board in conjunction with his Application for Accidental Disability Retirement from the Public Safety Personnel Retirement System (PSPRS). B. Medical Evaluations for two (2) New Police Personnel were reviewed by the Board: Hilda Galindo Tyler Michael Laufer 7. ADJOURN EXECUTIVE SESSION AND RECONVENE IN OPEN SESSION 8. ACCIDENTAL DISABILITY RETIREMENT APPLICATION The Board acknowledges that Police Officer Wesley Zygmont has filed an Application for Accidental Disability Retirement from the Public Safety Personnel Retirement System (PSPRS), and has directed that an Independent Medical Examination be conducted. CONSIDERATION AND POSSIBLE VOTE ON EXECUTIVE SESSION ITEMS Police Officer Michele Blanco confirmed that the following new Police Personnel met the standards of the Glendale Police Department and the Arizona Peace Officer Standards and Training Board: Hilda Galindo Tyler Michael Laufer • Motioned by Board Member Justin Harris, seconded by Board Member Cody Carmichael AYE: Mayor Jerry Weiers Board Member Cody Carmichael Board Member Charles Erickson Board Member Justin Harris Board Member Kevin Phelps Passed 10. BOARD COMMENTS AND SUGGESTIONS There were no Board comments or suggestions. 11. NEXT MEETING The next regular meeting of the Public Safety Personnel Retirement System Local Police Board will be held on November 21, 2018 at 9:30 a.m., in the 4th Floor Conference Room, located at 5850 W. Glendale Ave., Glendale, Arizona, 85301. 12.
