Call to Order — Police Board
Charles Erickson, Acting Chair, called to order the Local Police Board meeting at 9:07 a.m.
Review and Approval of the February 22, 2017 Local Police Board Meeting Minutes
The minutes of the February 22, 2017 Local Police Board meeting were reviewed. Officer Justin Harris
moved to accept the minutes as presented. Officer Cody Carmichael seconded the motion. The Board voted
and passed the motion unanimously.
Distribution of the June 30, 2016 Annual Actuarial Valuation of Members for Glendale Police Covered
By the PSPRS
Charles Erickson, Acting Chair, acknowledged that board members received, in their board books, the June
30, 2016 Annual Actuarial Valuation report of Glendale Police covered by the PSPRS.
Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan
("D.R.O.P.") — John G. Bustoz
The Local Police Board reviewed John G. Bustoz's D.R.O.P. Enrollment Application. John G. Bustoz, Police
Officer, has 25.003 years of credited service. He elected to participate in the D.R.O.P. on April 15, 2017. He
must terminate employment with the City of Glendale Police Department on or before April 30, 2022. He has
been a member of the Glendale Police Department since April 15, 1992. The action requested of the Board is
to approve the Application for Enrollment into D.R.O.P. Retirement submitted by John G. Bustoz, Police
Officer.
Officer Justin Harris moved to approve the Application for Enrollment in D.R.O.P. Retirement submitted by
John G. Bustoz, Police Officer. Officer Cody Carmichael seconded the motion. The motion passed
unanimously.

Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan

The Local Police Board reviewed Julie Anne Pendergast's D.R.O.P. Enrollment Application. Julie Anne
Pendergast, Police Officer, has 25.008 years of credited service. She elected to participate in the D.R.O.P. on
April 13, 2017. She must terminate employment with the City of Glendale Police Department on or before
April 30, 2022. She has been a member of the Glendale Police Department since April 13, 1992. The action
requested of the Board is to approve the Application for Enrollment into D.R.O.P. Retirement submitted by
Julie Anne Pendergast, Police Officer.
Officer Justin Harris moved to approve the Application for Enrollment in D.R.O.P. Retirement submitted by
Julie Anne Pendergast, Police Officer. City Manager Kevin Phelps seconded the motion. The motion passed
unanimously.
Review and Approval of Application for Normal Retirement — Hans J. Unvericht
The Local Police Board reviewed Hans J. Unvericht's Application for Normal Retirement. Hans J. Unvericht,
Police Officer, has completed 17.441 years of credited service. He is scheduled to retire on September 17,
2017. He has been a member of the Public Safety Personnel Retirement System (PSPRS) since April 10,
2000. The action requested of the Board is to approve the Normal Retirement Application for Hans J.
Unvericht, Police Officer.
Officer Justin Harris made a motion to approve the Application for Normal Retirement submitted by Hans J.
Unvericht, Police Officer. Officer Cody Carmichael seconded the motion. The Board voted and passed the
motion unanimously.
Review and Approval of Application for Survivor's Benefits — Donna White
The Local Police Board reviewed the Application for Survivor's Benefits submitted by Donna White, the
spouse of J.L. White, deceased Police Officer. The title of Police Officer was given to our HR Benefits
Administrator, Vicki Moss, by the PSPRS Administration for the Survivor's Benefits documents. However,
Charles Erickson, Citizen Board Member and Acting Chair, advised that he had known Mr. White for a long
time and Mr. Erickson requested that the minutes reflect that J.L. White was a Police Captain when he retired.
Donna White submitted the Application in accordance with the Arizona Revised Statute 38-846A. J.L. White
passed away on March 5, 2017.
Officer Justin Harris moved to approve the Application for Survivor's Benefits submitted by Donna White.
Officer Cody Carmichael seconded the motion. The Board voted and passed the motion unanimously.
Commencement of Executive Session — 9:11 a.m.
Charles Erickson, Acting Chair, asked for a motion to enter into Executive Session. Mayor Jerry Weiers made
the motion and Officer Justin Harris seconded the motion. The motion was unanimous.
Review and Consider Acceptance of Three (3) New Police Personnel into the Public Safety Personnel
Retirement System (PSPRS)
The Board reviewed the medical evaluations submitted for the three (3) new Police Personnel.

Adjourn Executive Session — 9:25 a.m.
Charles Erickson, Acting Chair, asked for a motion to adjourn Executive Session and reconvene in Open
Session. Officer Justin Harris made the motion and City Manager Kevin Phelps seconded the motion. The
motion was unanimous.
Consider and Vote on Admission of Three (3) New Police Personnel for Membership into the PSPRS
with Any Pre -Existing Conditions Noted
Police Officer Michele Blanco stated for the record that the following new Police personnel met the standards
of the Glendale Police Department and the Arizona Peace Officer Standards and Training Board:

Alejandro Mendez
Amanda Lynn Samudio
Charles Erickson, Acting Chair, asked for a motion to accept the three (3) applications for the new Police
personnel. Officer Cody Carmichael made a motion to accept the following new Police personnel into the
PSPRS with any pre-existing conditions noted:

Alejandro Mendez
Amanda Lynn Samudio
Officer Justin Harris seconded the motion. The Board voted and passed the motion unanimously.

There was no new business.

Charles Erickson, Acting Chair, asked for a, motion to adjourn the meeting. Officer Justin Harris made a
motion to adjourn the meeting. Officer Cody Carmichael seconded the motion. The Local Police Board
meeting adjourned at 9:27 a.m.
