Call to Order Local Fire Board Meetine
Charles Erickson, Acting Chair, called to order the Local Fire Board meeting at 9:02 a.m.
Review and Apnroval of the October 26.2016 Local Fire Board Meetine Minutps
The minutes of the October 26,2016 Local Fire Board meeting were reviewed. Capt. Anthony Butch
moved to accept the minutes as presented. Capt. Joe Hester seconded the motion. The Board voted and
passed the motion unanimously.
Distribution of the June 30.2015 Annual Actuarial Valuation of Members for Glendale Fire
Covered Bv the PSPRS
Charles Erickson, Acting Chair, acknowledged that board members received, in their board books, the
June 30,2015 Annual Actuarial Valuation report of Glendale Fire Fighters covered by the PSPRS.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.') - Steve Matousek
The Local Fire Board reviewed Steve Matousek's D.R.O.P. Retirement Application. Steve Matousek,
Fire Fighter, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Fire
Department on January 7,2017. On March 28,2012, the Local Fire Board had approved Steve Matousek
into the D.R.O.P. At that time, he had completed 25.004 years of credited service. He was required to
terminate employment with the City of Glendale Fire Department on or before February 28,2017 . Steve
Matousek is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution. He will begin
receiving his normal retirement pension on or about February 28, 2017. The action requested of the

Board is to approve the Application for Retirement from the D.R.O.P. and lump sum distribution for
Steve Matousek, Fire Fighter.
Capt. Anthony Butch moved to approve the Application for Retirement from the D.R.O.P. submitted by
Steve Matousek, Fire Fighter. City Manager Kevin Phelps seconded the motion. The Board voted and
passed the motion unanimously.
Reemployment Determination for Two (2) Retired Fire Personnel
Jim Brown, Board Secretary, advised the Board that there were two (2) Reemployment Determinations
that needed to be addressed by the Board. Mr. Brown read the A.R.S. state statute Section $38-849 D
which was in effect from 2002-2008, the time period in which the reemployments took place. This statute
stated "If a retired member is reemployed by an employer, no contributions shall be made on the retired
member's account, nor any service credited, during the period of such reemployment. In addition, if a
retired member subsequently becomes employed in the same position by the employer from which the
member retired, the system shall not make pension payments to the retired member during the period of
reemployment. On subsequent termination of employment by the retired member, the retired member is
entitled to receive a pension based on the member's service and compensation before the date of the
member's reemployment. If a member who retired under disability is reemployed by an employer as an
employee, that member shall be treated as if the member had been on an uncompensated leave of absence
during the period of the member's disability retirement and shall be a contributing member of the
system."
Reemnlovment Determination - Retired Fire Eneineer Donald Jessee
Donald Jessee, retired Fire Engineer, was rehired by the City to the position of Equipment Mechanic
Specialist. Mr. Brown provided the Board with the job desoiptions for both Fire Engineer and
Equipment Mechanic Specialist and explained that the PSPRS Board must determine and agree that
retired Fire Engineer; Donald Jessee, was not rehired into the same position from which he retired. The
Board reviewed the job descriptions for both Fire Engineer and Equipment Mechanic Specialist. The
Board determined that Mr. Jessee was not rehired in the same position from which he retired.
Charles Erickson, Acting Chair, asked for a motion and a second on the retroactive reemployment
determination of retired Fire Engineer Donald Jessee. City Manager Kevin Phelps made the motion that
retired Fire Engineer Donald Jessee was not rehired by the City in the same position from which he
retired. Capt. Anthony Butch seconded the motion. The motion passed unanimously.
Reemplovment Determination - Retired Fire Captain Wavne Smith
Jim Brown, Board Secretary, requested that the Board table this Reemployment Determination of retired
Fire Captain Wayne Smith who was rehired by the City to the position of Fire Battalion Chief. Mr.
Brown would like to provide additional information to the Board so that they will have all documents
needed to respond to the state statute questions with regard to Mr. Smith's reemployment by the City.
Charles Erickson, Acting Chair, asked for a motion to table the reemployment determination of retired
Fire Captain Wayne Smith. City Manager Kevin Phelps made the motion to table the reemployment
determination of retired Fire Captain Wayne Smith. Capt. Joe Hester seconded the motion. The motion
passed unanimously.

Commencement of Executive Session - 9:25 a.m.
Charles Erickson, Acting Chair, asked for a motion to recess the Open Session and convene into
Executive Session to review and discuss the medical evaluations of seven (7) new Fire personnel. City
Manager Kevin Phelps made a motion to go into Executive Session. Capt. Anthony Butch seconded the
motion.
Adiourned Executive Session and Reconvened into Open Session - 9:30 a.m.
Charles Erickson, Acting Chair, asked for a motion to adjourn Executive Session and reconvene in Open
Session. Capt. Anthony Butch made a motion to adjourn Executive Session and to reconvene into Open
Session. City Manager Kevin Phelps seconded the motion.
Charles Erickson, Acting Chair asked for a motion to accept the seven (7) applications for the new Fire
personnel. Capt. Joe Hester made a motion to accept the applications of the following new Fire personnel
into the PSPRS with any pre-existing conditions noted:
Timothy Benson
Nicholas Haney
Michael Harris
Stephen Orsini
Kurt Schupe
Justin Smith
Kyle Taylor
City Manager Kevin Phelps seconded the motion. The Board voted and passed the motion unanimously.
New Business
Jim Brown, Board Secretary, advised the Board that there are changes being proposed in the state
legislature regarding consolidation of local boards that have 150 members or less. They would be
consolidated into one either county or state board. This should not have an impact on the City of
Glendale since our Fire and Police membership is greater than 150. Should we hear of consolidation of
local boards, it would not have an impact on the City of Glendale. Charles Erickson, Acting Chair, stated
that, as he read the information, the city could decide to join a larger board but in doing so, it would take
all control away from the city. Mayor Jerry Weiers commented that he does not see any benefit to the city
in joining a larger board. The Board members all agreed.

Charles Erickson, Acting Chair, asked for a motion to adjourn the meeting. Capt. Joe Hester made a
motion to adjoum the Fire Board meeting. City Manager Kevin Phelps seconded the motion. The
meeting was adjourned at 9:32 a.m.
