AZ Charles Erickson, Citizen Board Member and Acting Chair Police Offrcer Justin Harris, Board Member Police Offrcer Cody Carmichael, Board Member Jim Brown, Director Human Resources and Board Secretary Nancy Mangone, Assistant City Attorney Michele Blanco, Police Offrcer Vicki Moss, HR Administrator, Benefits Marcia Zientek, Human Resources Support Staff Mayor Jerry Weiers, Board Member, Chair City Manager Kevin Phelps, Board Member Call to Order- Police Board Charles Erickson, Acting Chair, called to order the Local Police Board meeting at 9:09 a.m. Review and Approval of the Aueust 24.2016 Local Police Board Meetine Minutes The minutes of the August 24,2016 Local Police Board meeting were reviewed. Officer Justin Hanis moved to accept the minutes as presented. Officer Cody Carmichael seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan ("D.R.O.P.") - Donald Alan Kralovetz The Local Police Board reviewed Donald Alan lftalovetz's D.R.O.P. Enrollment Application. Donald Alan Kralovetz, Police Sergeant, has 26.008 years of credited service. He elected to participate in the D.R.O.P. on September 12,2016. He chose to participate in the D.R.O.P. for a period of sixty (60) months. He must terminate employment with the City of Glendale Police Deparfinent on or before September 30,2021. He has been a member of the Glendale Police Department since September 10, 1990. The action requested of the Board is to approve the Application for Enrollment into D.R.O.P. Retirement submitted by Donald Alan Kralovetz, Police Sergeant. Officer Justin Hanis moved to approve the Application for Enrollment in D.R.O.P. Retirement submitted by Donald Alan Kralovetz, Police Sergeant. Officer Cody Carmichael seconded the motion. The motion passed unanimously. Page I of4 PSPRS Local Police Board Meeting Minutes 9/28t16 Review and Aoproval of Annlication for Retirement from the Deferred Retirement Ontion Plan ("D.RO.P.") - Judy Ann Mannins The Local Police Board reviewed Judy Ann Manning's D.R.O.P. Retirement Application. Judy Ann Manning, Police Lieutenant, has elected to retire from the D.R.O.P. and terminate her employment with the Glendale Police Department on December 31, 2016. On November 16, 2011, the Local Police Board had approved an Application for Enrollment into D.R.O.P. effective December 30, 2011. At that time, she had completed 25.819 years of credited service. She was required to terminate employment with the City of Glendale Police Department on or before December 31,2016. Judy Ann Manning is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution. She will begin receiving her normal retirement pension on or about January 31,2017. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and Lump Sum distribution for Judy Ann Manning, Police Lieutenant. Officer Justin Harris moved to approve the Application for Retirement from the D.R.O.P. submitted by Judy Ann Manning, Police Lieutenant. Officer Cody Carmichael seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of Application for Retirement from the Deferred Retirement Option Plan (*D.RO.P.") - Michael D. Michelsen The Local Police Board reviewed Michael D. Michelsen's D.R.O.P. Retirement Application. Michael D. Michelsen, Police Officer, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Police Department on December 23,2016. On November 16,2011, the Local Police Board had approved an Application for Enrollment into D.R.O.P. effective December l,20ll. At that time, he had completed 25.425 years of credited service. He was required to terminate employment with the City of Glendale Police Department on or before December 31,2016. Michael D. Michelsen is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution. He will begin receiving his normal retirement pension on or about January 31,2017. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and Lump Sum distribution for Michael D. Michelsen, Police Officer. Officer Justin Harris moved to approve the Application for Retirement from the D.R.O.P. submitted by Michael D. Michelsen, Police Officer. Offrcer Cody Carmichael seconded the motion. The Board voted and passed the motion unanimously. Review and Apnroval of Application for Retirement from the Deferred Retirement Ontion Plan ("D.R.O.P.D) - Jaime Nowatzki The Local Police Board reviewed Jaime Nowatzki's D.R.O.P. Retirement Application. Jaime Nowatzki, Police Officer, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Police Department on December 18, 2016. On May 25,201l, the Local Police Board had approved an Application for Enrollment into D.R.O.P. effective December 18, 201l. At that time, he had completed 30.003 years of credited service. He was required to terminate employment with the City of Glendale Police Department on or before December 31,2016. Jaime Nowatzki is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution. He will begin receiving his normal retirement pension on or about January 31,2017. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and Lump Sum distribution for Jaime Nowatzki, Police Officer. Officer Cody Carmichael seconded the motion. The Board voted and passed the motion unanimously. Apolication for Disabilitv Retirement from the Public Safety Personnel Retirement Svstem (PSPRS) - Notification - Korv Raffertv The Board was advised that an Application for Disability Retirement from the PSPRS was received from Kory Rafferty, Police Officer. The full application and all supporting documentation will be presented to the Board at the October 26,2016 Local Police Board meeting. Commencement of Executive Session - 9:15 a.m. Charles Erickson, Acting Chafu, asked for a motion to enter into Executive Session. Officer Justin Hanis made the motion and Officer Cody Carmichael seconded the motion. The motion was unanimous. Review and Consider Acceptance of Three (3) New Police Personnel into the Public Safetv Personnel Retirement Svstem (PSPRS) The Board reviewed the medical evaluations submitted for the three (3) new Police Personnel. Adiourn Executive Session - 9:18 a.m. Charles Erickson, Acting Chair, asked for a motion to adjourn Executive Session and reconvene in Open Session. Oflicer Justin Hanis made the motion and Officer Cody Carmichael seconded the motion. The motion was unanimous. Consider and Vote on Adminssion of Three (3) New Police Personnel for Membership into the PSPRS with Anv Pre-Existins Conditions Noted Police Officer Michele Blanco stated for the record that each of the following new personnel met the standards of the Glendale Police Department and the Arizona Peace Officer Standards and Training Board: Christopher Bonadie Jacob Gonzales Jason Eric Martinez Officer Justin Hanis made a motion to accept the applications of the following Police personnel into the PSPRS with any pre-existing conditions noted: Christopher Bonadie Jacob Gonzales Jason Eric Martinez Offrcer Cody Carmichael seconded the motion. The Board voted and passed the motion unanimously. New Business There was no new business.
