Call to Order—Police Board
Charles Erickson, Acting Chair, called to order the Local Police Board meeting at 9:09 a.m.
Review and Approval of the April 27, 2016 Local Police Board Meeting Minutes
The minutes of the April 27, 2016 Local Police Board meeting were reviewed. Mayor Jerry Weiers moved to
accept the minutes as presented. Officer Cody Carmichael seconded the motion. The Board voted and passed
the motion unanimously.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.")—John Babinchak
The Local Police Board reviewed John Babinchak's D.R.O.P. Retirement Application. John Babinchak,
Police Officer, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Police
Department on July 7, 2016. On May 25, 2011, the Local Police Board had approved an Application for
Enrollment in D.R.O.P. effective July 8, 2011. At that time, he had completed 20.022 years of credited
service. He was required to terminate employment with the City of Glendale Police Department on or before
July 31, 2016. John Babinchak is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution.
He will begin receiving his normal retirement pension on or about August 31, 2016. The action requested of
the Board is to approve the Application for Retirement from the D.R.O.P. and Lump Sum distribution for John
Babinchak, Police Officer.
Officer Cody Carmichael moved to approve the Application for Retirement from the D.R.O.P. submitted by
John Babinchak, Police Officer. City Manager Kevin Phelps seconded the motion. The Board voted and
passed the motion unanimously.

Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.")—Willie A. Jackson
The Local Police Board reviewed Willie A. Jackson's D.R.O.P. Retirement Application. Willie A. Jackson,
Police Officer, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Police
Department on June 1, 2016. On January 22, 2014, the Local Police Board had approved an Application for
Enrollment in D.R.O.P. effective November 29, 2013. At that time, he had completed 20.099 years of
credited service. He was required to terminate employment with the City of Glendale Police Department on or
before November 30, 2018. Willie A. Jackson is eligible to retire from the D.R.O.P. and to receive a Lump
Sum distribution. He will begin receiving his normal retirement pension on or about July 31, 2016. The
action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and Lump Sum
distribution for Willie A. Jackson, Police Officer.
Officer Cody Carmichael moved to approve the Application for Retirement from the D.R.O.P. submitted by
Willie A. Jackson, Police Officer. City Manager Kevin Phelps seconded the motion. The Board voted and
passed the motion unanimously.
Commencement of Executive Session—9:11 a.m.
Charles Erickson, Acting Chair, asked for a motion to enter into Executive Session. Mayor Jerry Weiers made
the motion and Officer Cody Carmichael seconded the motion. The motion was unanimous.
Review and Consider Acceptance of Six (6) New Police Personnel into the Public Safety Personnel
Retirement System (PSPRS)
The Board reviewed the medical evaluations submitted for the six (6) new Police Personnel.
Adjourn Executive Session—9:18 a.m.
Charles Erickson, Acting Chair, asked for a motion to adjourn Executive Session and reconvene in Open
Session. City Manager Kevin Phelps made the motion and Officer Cody Carmichael seconded the motion.
The motion was unanimous.
Consider and Vote on Adminssion of Six (6) New Police Personnel for Membership into the PSPRS with
Any Pre-Existing Conditions Noted
Police Officer Michele Blanco stated for the record that each of the following new personnel met the standards
of the Glendale Police Department and the Arizona Peace Officer Standards and Training Board:
Destiny-Rene Ebersohl Andrew Ellis Johnson
Brooke Mauresa Hanley Anthony Michael Johnson
Kyle James Hansen Jason Bradley Kline
Officer Cody Carmichael made a motion to accept the applications of the following Police personnel into the
PSPRS with any pre-existing conditions noted:
Destiny-Rene Ebersohl Andrew Ellis Johnson
Brooke Mauresa Hanley Anthony Michael Johnson
Kyle James Hansen Jason Bradley Kline

City Manager Kevin Phelps seconded the motion. The Board voted and passed the motion unanimously.
New Business
There was no new business.
Adjournment
Charles Erickson, Acting Chair, asked for a motion to adjourn the meeting. Officer Cody Carmichael made a
motion to adjourn the meeting. City Manager Kevin Phelps seconded the motion. The Local Police Board
meeting adjourned at 9:21 a.m.
