Glendale Ave. Glendale, AZ MEMBERS PRESENT: Mayor Jerry Weiers, Board Member, Chair Charles Erickson, Board Member and Acting Chair City Manager Kevin Phelps, Board Member Capt. Joe Hester, Board Member Capt. Anthony Butch, Board Member STAFF PRESENT: Jim Brown, Director Human Resources and Board Secretary Nancy Mangone, Assistant City Attorney Vicki Moss, Benefits Administrator Chris DeChant, Assistant Fire Chief Marcia Zientek, Human Resources Support Staff Call to Order Local Fire Board Meeting Charles Erickson, Acting Chair, called to order the Local Fire Board meeting at 9:03 a.m. Review and Approval of the April 27,2016 Local Fire Board Meeting Minutes The minutes of the April 27, 2016 Local Fire Board meeting were reviewed. Mayor Jerry Weiers moved to accept the minutes as presented. Capt. Anthony Butch seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of Application for Retirement from the Deferred Retirement Option Plan ("D.R.O.P.")—Richard Ellis Franklin The Local Fire Board reviewed Richard Ellis Franklin's D.R.O.P. Retirement Application. Richard Ellis Franklin, Fire Captain, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Fire Department on June 29, 2016. On June 22, 2011, the Local Fire Board had approved Richard Ellis Franklin into the D.R.O.P. effective June 1, 2011. At that time, he had completed 25.326 years of credited service. He was required to terminate employment with the City of Glendale Fire Department on or before June 30, 2016. Richard Ellis Franklin is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution. He will begin receiving his normal retirement pension on or about July 31, 2016. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and lump sum distribution for Richard Ellis Franklin, Fire Captain. Capt. Joe Hester moved to approve the Application for Retirement from the D.R.O.P. submitted by Richard Ellis Franklin, Fire Captain. City Manager Kevin Phelps seconded the motion. The Board voted and passed the motion unanimously. Walter II The Local Fire Board reviewed John A. Walter II's D.R.O.P. Retirement Application. John A.Walter II, Fire Captain, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Fire Department on June 24, 2016. On June 22, 2011, the Local Fire Board had approved John A. Walter II into the D.R.O.P. effective June 30, 2011. At that time, he had completed 26.058 years of credited service. He was required to terminate employment with the City of Glendale Fire Department on or before June 30, 2016. John A. Walter II is eligible to retire from the D.R.O.P. and to receive a Lump Sum distribution. He will begin receiving his normal retirement pension on or about July 31, 2016. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and lump sum distribution for John A.Walter II, Fire Captain. Capt. Anthony Butch moved to approve the Application for Retirement from the D.R.O.P. submitted by John A. Walter II, Fire Captain. City Manager Kevin Phelps seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of the Application for Survivor's Benefits From the Public Safety Personnel Retirement System (PSPRS)—Lou E. Ghering The Local Fire Board reviewed the Application for Survivor's Benefits for Lou E. Ghering, the spouse of William E. Ghering, retired Glendale Fire employee, deceased. William E. Ghering passed away on April 21, 2016. Lou E. Ghering submitted the Application in accordance with the Arizona Revised Statute 38- 846(A). The action requested of the Board is to approve the Application for Survivor's Benefits from the PSPRS submitted by Lou E. Ghering, spouse of deceased Glendale Fire employee, William E. Ghering. Capt. Anthony Butch moved to approve the Application for Survivor's Benefits submitted by Lou E. Ghering. City Manager Kevin Phelps seconded the motion. The Board voted and passed the motion unanimously. New Business There was no new business. Adjournment Charles Erickson, Acting Chair, asked for a motion to adjourn the meeting. Mayor Jerry Weiers made a motion to adjourn the Fire Board meeting. City Manager Kevin Phelps seconded the motion.
