Call to Order
Charles Erickson, Acting Chair, called to order the.Local Police Board meeting at 9:07 a.m.
Review and Approval of the November 18,2015 Local Police Board Meeting Minutes
The minutes of the November 18, 2015 Local Police Board meeting were reviewed. Mayor Jerry Weiers
moved to accept the minutes as presented. Officer Cody Carmichael seconded the motion. The Board voted
and passed the motion unanimously.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.")—Dawn Gallagher
The Local Police Board reviewed Dawn Gallagher's D.R.O.P. Retirement Application. Dawn Gallagher,
Police Sergeant, has elected to retire from the D.R.O.P. and terminate her employment with the Glendale
Police Department on January 29, 2016. On July 25, 2012, the Local Police Board had approved an
Application for Enrollment in D.R.O.P. submitted by Dawn Gallagher. At that time, she had completed
21.268 years of credited service. She was required to terminate employment with the City of Glendale Police
Department on or before June 30, 2017. Dawn Gallagher is eligible to retire from the D.R.O.P. and to receive
a Lump Sum distribution. She will begin receiving her normal retirement pension on or about February 29,
2016. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P. and
Lump Sum distribution for Dawn Gallagher, Police Sergeant.

Officer Cody Carmichael moved to approve the Application for Retirement from the D.R.O.P. submitted by
Dawn Gallagher, Police Sergeant. Mayor Jerry Weiers seconded the motion. The Board voted and passed the
motion unanimously.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.")—Matthew M. Lively
The Local Police Board reviewed Matthew M. Lively's D.R.O.P. Retirement Application. Matthew M.
Lively, Assistant Police Chief, has elected to retire from the D.R.O.P. and terminate his employment with the
Glendale Police Department on February 26, 2016. On April 27, 2011, the Local Police Board had approved
an Application for Enrollment in D.R.O.P. submitted by Matthew M. Lively. At that time, he had completed
25.263 years of credited service. He was required to terminate employment with the City of Glendale Police
Department on or before February 25, 2016. Matthew M. Lively is eligible to retire from the D.R.O.P. and to
receive a Lump Sum distribution. He will begin receiving his normal retirement pension on or about March
31, 2016. The action requested of the Board is to approve the Application for Retirement from the D.R.O.P.
and Lump Sum distribution for Matthew M. Lively, Assistant Police Chief.
Officer Cody Carmichael moved to approve the Application for Retirement from the D.R.O.P. submitted by
Matthew M. Lively, Assistant Police Chief. Mayor Jerry Weiers seconded the motion. The Board voted and
passed the motion unanimously.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.")—Genea Stephens
The Local Police Board reviewed Genea Stephens' D.R.O.P. Retirement Application. Genea Stephens, Police
Sergeant, has elected to retire from the D.R.O.P. and terminate her employment with the Glendale Police
Department on January 22, 2016. On August 26, 2015, the Local Police Board had approved an Application
for Enrollment in D.R.O.P. submitted by Genea Stephens. At that time, she had completed 20.024 years of
credited service. She was required to terminate employment with the City of Glendale Police Department on
or before July 31, 2020. Genea Stephens is eligible to retire from the D.R.O.P. and to receive a Lump Sum
distribution. She will begin receiving her normal retirement pension on or about February 29, 2016. The
action requested of the,Board is to approve the Application for Retirement from the D.R.O.P. and Lump Sum
distribution for Genea Stephens, Police Sergeant.
Officer Cody Carmichael moved to approve the Application for Retirement from the D.R.O.P. submitted by
Genea Stephens, Police Sergeant. Mayor Jerry Weiers seconded the motion. The Board voted and passed the
motion unanimously.
Review and Approval of Application for Survivor's Benefits -Derek Johnson
The Local Police Board reviewed the Application for Survivor's Benefits submitted by Derek Johnson, the
spouse of LeeAnn Johnson, deceased Police Officer. Derek Johnson submitted the Application in accordance
with the Arizona Revised Statute 38-846. LeeAnn Johnson passed away on December 26, 2015.
Officer Cody Carmichael moved to approve the Application for Survivor's Benefits submitted by Derek
Johnson. Mayor Jerry Weiers seconded the motion. The Board voted and passed the motion unanimously.

Commencement of Executive Session
Charles Erickson, Acting Chair, recessed the Open Session and convened into Executive Session to review
and discuss two (2) items:
1) Review and Consider Acceptance of Six (6) New Police Personnel into the Public Safety Personnel
Retirement System (PSPRS).
2) Review and Consider the Application for Accidental Disability Retirement for One (1) Police Officer.
Reconvened into Open Session
The Board adjourned Executive Session and reconvened into Open Session to vote on the two (2) items:
1. Consider and Vote on the admission of six(6) new Police personnel into the PSPRS with any pre-
existing conditions noted on the medical evaluations.
Officer Michele Blanco affirmed that the following Police personnel met the standards of the Glendale
Police Department and the Arizona Peace Officer Standards and Training Board:

Cody Tyler Harris Karina Sanders
Laura Marissa Lechuga Maxwell Andersen Turner
Officer Cody Carmichael made a motion to accept the applications of the six (6) new Police personnel
into the PSPRS with any pre-existing conditions noted. Mayor Jerry Weiers seconded the motion. The
Board voted and passed the motion unanimously.
2. Consider and Vote on the Application for Accidental Disability Retirement from the Public
Safety Personnel Retirement System ("PSPRS")—Ryan Brady
Ryan Brady, a Police Officer with the City of Glendale, applied for Accidental Disability Retirement
through the Public Safety Personnel Retirement System ("PSPRS") on June 10, 2015. He has been a
member of the PSPRS since March 28, 2005. On July 26, 2005, he was accepted into the Public
Safety Personnel Retirement System.
At this time, Mr. Jim Brown, PSPRS Board Secretary, read the requirements of the eight (8) discussion
questions that the Board was to address. This discussion would become part of the board minutes.
1. Did the employee file the application after the disabling incident, or within one (1) year of ceasing
to be an employee?
Mr. Brown stated that, from an HR standpoint, the answer to that is `yes'.
2. Did he (or will)the employee terminate by reason of disability?
Mr. Brown noted that, as the board members read in the Executive Session, the answer to that
would be `yes'.

3. Did employment terminate based on a disciplinary issue?
The answer to this question is 'no'
4. Did employment terminate based on the employee's participation in DROP?
'No'. Employment was not terminated based on a DROP Enrollment.
5. Is the employee still working a position within their job classification that the Local Board
considers a reasonable range of duties?
The answer is 'no'.
6. Has the employee refused a position within their job classification that the Local Board considers a
reasonable range of duties position?
The answer is 'no'.
7. Did injury or condition occur prior to the current PSPRS membership date?
'No'. It occurred during the membership date as was indicated in the files.
8. Was the injury or condition the result of an event incurred during the performance of the
employee's duty?
The answer to that is `yes'.
Charles Erickson, Acting Chair, then asked Mr. Brown to read through the three (3) statutory questions
that the Board must vote upon.
1. Does Mr. Brady have a physical or mental condition which totally and permanently prevents him
from performing a reasonable range of duties within his job classification as a Police Officer?
The Board unanimously answered `yes'.
2. Was Mr. Brady's disabling condition or injury incurred in the performance of Mr. Brady's duty as
a Police Officer?
The Board unanimously answered `yes'.
3. Does Mr. Brady's disability result from a physical or mental condition or injury that existed or
occurred prior to his date of membership in the PSPRS which was July 26, 2005?
The Board unanimously answered 'no'.
Since all the statutory questions were answered, Charles Erickson, Acting Chair, asked for a motion to
accept Mr. Brady's Application for Accidental Disability Retirement. Officer Cody Carmichael made
a motion to approve the Application for Accidental Disability Retirement from the Public Safety
Personnel Retirement System submitted by Ryan Brady. Mayor Jerry Weiers seconded the motion.
The Board unanimously approved the application.

There was no new business.
Adjournment
Charles Erickson, Acting Chair, asked for a motion to adjourn the meeting. Mayor Jerry Weiers made a
motion to adjourn the meeting. Officer Cody Carmichael seconded the motion. The meeting was adjourned at
9:40 a.m.
