Charles Erickson, Citizen Board Member and Acting Chair
Police Officer Justin Harris, Board Member

Call to Order
Charles Erickson, Acting Chair, called to order the Local Police Board meeting at 9:21 a.m.
Review and Approval of the October 28,2015 Local Police Board Meeting Minutes
The minutes of the October 28, 2015 Local Police Board meeting were reviewed. Mayor Jerry Weiers moved
to accept the minutes as presented. Officer Justin Harris seconded the motion. The Board voted and passed
the motion unanimously.
Review and Approval of Application for Normal Retirement—Phillis Walker
The Local Police Board reviewed Phillis Walker's Application for Normal Retirement. Phillis Walker, Police
Officer, has completed 20.014 years of credited service. She is scheduled to retire on January 26, 2016. She
has been a member of the Public Safety Personnel Retirement System (PSPRS) since January 22, 1996. The
action requested of the Board is to approve the Normal Retirement Application for Phillis Walker, Police
Officer.
Officer Justin Harris made a motion to approve the Application for Normal Retirement submitted by Phillis
Walker, Police Officer. Mayor Jerry Weiers seconded the motion. The Board voted and passed the motion
unanimously.

Acknowledgement of Filing for Disability Retirement—Ryan Owen Brady
The Board was notified and asked to acknowledge that an Application for Disability Retirement has been filed
by Ryan Owen Brady, Police Officer. Officer Brady's full application will come before the board at a later
date.
Commencement of Executive Session
Charles Erickson, Acting Chair, recessed the Open Session and convened into Executive Session to review
and discuss the medical evaluations of three (3) new Police personnel.
Reconvened into Open Session
The Board adjourned Executive Session and reconvened into Open Session to vote on the admission of two
(2) of the three (3) new Police personnel into the PSPRS with any pre-existing conditions noted on the medical
evaluations.
Due to a conflict of interest regarding a quorum for the third person's membership, the application was tabled.
This application will be presented at the next PSPRS Board meeting when there will be the necessary quorum
to vote on that person's membership into the PSPRS.
Officer Michele Blanco affirmed that Carlos Rosario Bravo and Elaine Singer both met the standards of the
Glendale Police Department and the Arizona Peace Officer Standards and Training Board.
Officer Justin Harris made a motion to accept the applications of two (2) of the three (3) new Police personnel
into the PSPRS with any pre-existing conditions noted:
Carlos Rosario Bravo
Elaine Singer
Mayor Jerry Weiers seconded the motion. The Board voted and passed the motion unanimously.

There was no new business.

Charles Erickson, Acting Chair, asked for a motion to adjourn the meeting. Officer Justin Harris made a
motion to adjourn the meeting. Mayor Jerry Weiers seconded the motion. The meeting was adjourned at 9:27
a.m.
