Maryland Ave No. B Glendale, AZ 85301 Request for New Sewer Easement for the Sewer and Manhole Rehabilitation Project Phase IV Dear Robert Lane Revocable Trust: This letter is to inform the owner(s) of the property located at 6214 W. Maryland Ave, Glendale, AZ 85301, is located within an area of construction along a new sewer alignment that is part of the City of Glendale's Sewer and Manhole Rehabilitation Project Phase W. Previous notifications were issued to request a new sewer easement on this property, as shown on the attached exhibits. This letter is sent from the City of Glendale Engineering Department to have several homeowners located within the construction area to sign and notarize the required easement declaration with legal exhibit that will allow the City to remove, or abandon, a dilapidated portion of sewer main that exists on, or near your property. This new sewer easement will allow the City access to maintain and install the new pipe. This new sewer pipe will replace a portion of the existing sewer line that had frequent maintenance problems, produced foul odors, or created potential overflow issues. Your cooperation helps the City to fulfill its commitment to the community by improving the wastewater infrastructure. The City of Glendale ordinance requires all new sewer pipe to be installed within a sewer easement, when located on private property. Therefore, it is necessary for the City to record this sewer easement at the Maricopa County Recorder Office with a signed and notarized statement showing the legal description and exhibit of the new easement, located on the property. Following suggestions are ways to accomplish this: 1. Owner can sign and notarize the attached easement declaration in the space provided, same as the name shown. Then return the signed and notarized original to the City of Glendale in the envelope provided,before the deadline on May 19"'. 2. Owner can call Kathy Mitchell, a Notary Public within the City's Engineering Department, at(623) 930-3661, to schedule an appointment time to meet for signature and notarization prior to the deadline on May I9`h Construction is currently scheduled to begin in your area near the end of August. Redpoint Contracting is the contractor installing the new sewer pipe for this project. They will provide 3 days' notice prior to beginning construction within the sewer easement and 24-hour notice prior to entering the property.
