Call to Order
Charles Erickson, Acting Chair, called to order the Local Police Board meeting at 9:09 a.m.
Review and Approval of the August 27,2014 Local Police Board Meeting Minutes
The Minutes of the August 27, 2014 Local Police Board meeting were reviewed. Mayor Jerry Weiers moved
to accept the Minutes as presented. Officer Cody Carmichael seconded the motion. The Board voted and
passed the motion unanimously.
Application for Ordinary Disability Retirement from the Public Safety Personnel Retirement System
("PSPRS")—Evan Kudler
Jim Brown presented the application from Evan Kudler, a Police Officer with the City of Glendale, who has
applied for Ordinary Disability Retirement from the Public Safety Personnel Retirement System ("PSPRS").
Officer Evan Kudler had requested to attend the meeting and to be present during the Executive Session.
Officer Kudler was invited to join the meeting at this time, along with his wife, Geneen Kudler, and his
attorney, Kathryn Baillie.

Commencement of Executive Session
Charles Erickson, Acting Chair, recessed the Open Session and convened into Executive Session to review the
Application for Ordinary Disability Retirement from the PSPRS for Officer Evan Kudler.
At a point during the Executive Session, Mayor Weiers asked that the Board have an opportunity to obtain
legal counsel from Mr. DiPiazza, the Board's attorney; therefore, at that point, Officer Kudler, Geneen Kudler,
and Ms. Baillie were excused from the meeting.
After the Board's consultation with Mr. DiPiazza, Officer Kudler, Geneen Kudler and Ms. Baillie were invited
back into the meeting.
Reconvened into Open Session
The Board adjourned Executive Session and reconvened into Open Session to vote on the Application for
Ordinary Disability Retirement from the PSPRS for Officer Evan Kudler.
Mayor Jerry Weiers made a motion to have another examination performed by a new IME physician in the
appropriate specialty field. Mayor Weiers requested that, in addition to the physical examination, the new
physician be given all the medical records that have been completed up to now. Officer Justin Harris
seconded the motion.
Mr. DiPiazza addressed the issue brought up by Ms. Baillie, Mr. Kudler's attorney, regarding her complaint
about the errors she believes were done by the original IME doctor. Mr. DiPiazza asked that she write a letter
to the Human Resources Director, Jim Brown, specifically listing those errors and that the letter would be
included in the package that will be sent to the new IME physician. Ms. Baillie agreed to prepare and send a
letter to Mr. Brown with her concerns about the IME doctor and his report.
Mayor Weiers requested that the new examination be expedited. Mr. Brown advised that HR will do
everything possible to have it completed quickly; however, he stated that oftentimes HR must work with the
schedule of the IME physician. Mr. Brown stated that La Trisse Kuzinski and Lynn Baumer will be following
this direction and will have a new examination completed as promptly as possible.

Charles Erickson, Acting Chair, asked for a motion to adjourn the meeting. Mayor Jerry Weiers made a
motion to adjourn the meeting. Officer Justin Harris seconded the motion. The Local Police Board meeting
adjourned at 10:27 a.m.
