Call to Order A Joint Meeting of PSPRS Police and Fire Boards
Mayor Jerry Weiers called to order a joint meeting of the PSPRS Police and Fire Boards at 9:03 a.m.
Recognition of Service and Achievements of Larry Clark
Mayor Weiers presented Larry Clark, Citizen Board Member, with a plaque and card thanking him for his
eight years of service to both the PSPRS Police Board and the PSPRS Fire Board. Mayor Weiers recognized
the knowledge and commitment Mr. Clark brought to the board meetings and that he would certainly be
missed. Capt. Mark Manor presented Mr. Clark with a Challenge Coin on behalf of the Glendale Fire
Department. Mr. Clark responded that he has enjoyed his eight years as a member of the PSPRS Fire and
Police Boards and will miss everyone.
Adiournment
Mayor Weiers adjourned the joint meeting of the Police and Fire Boards. The joint meeting was adjourned at
9:10 a.m.

Mayor Weiers called to order the Local Police Board meeting at 9:15 a.m.

Review and Approval of the May 28, 2014 Local Police Board Meeting Minutes
The minutes of the May 28, 2014 Local Police Board meeting were reviewed. Lt. John Wolfe moved to
accept the Minutes as presented. Larry Clark seconded the motion. The Board voted and passed the motion
unanimously.
Review and Approval of Application for Normal Retirement — Dean M. Ferullo
The Local Police Board reviewed Dean M. Ferullo's Application for Normal Retirement. Dean M. Ferullo,
Police Officer, has completed 20.830 years of credited service. He is scheduled to retire on July 12, 2014. He
has been a member of the Public Safety Personnel Retirement System (PSPRS) since September 13, 1993.
The action requested of the Board is to approve the Normal Retirement Application for Dean M. Ferullo,
Police Officer.
Larry Clark made a motion to approve the Application for Normal Retirement submitted by Dean M. Ferullo,
Police Officer. Lt. John Wolfe seconded the motion. The Board voted and passed the motion unanimously.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.") — Brian K. Wilkins
The Local Police Board reviewed Brian K. Wilkins' D.R.O.P. retirement application. Brian K. Wilkins,
Police Officer, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Police
Department on July 31, 2014. On September 2, 2009, the Local Police Board had approved Brian K. Wilkins
into the D.R.O.P. He was required to terminate employment with the City of Glendale Police Department on
or before July 31, 2014. The action requested of the Board is to approve the application for retirement from
the D.R.O.P. and lump sum distribution for Brian K. Wilkins, Police Officer.
Lt. John Wolfe moved to approve the D.R.O.P. retirement application submitted by Brian K. Wilkins, Police
Officer. Larry Clark seconded the motion. The Board voted and passed the motion unanimously.
Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan
("D.R.O.P.") — John Robert Wolfe
The Local Police Board reviewed John Robert Wolfe's D.R.O.P. Enrollment Application. John Robert Wolfe,
Police Lieutenant, has 22.208 years of credited service. He has elected to participate in the D.R.O.P. on June
27, 2014. He must terminate employment with the City of Glendale Police Department on or before June 27,
2019. He has been a member of the Glendale Police Department since April 13, 1992. The action requested
of the Board is to approve the Application for Enrollment in D.R.O.P. Retirement submitted by John Robert
Wolfe, Police Lieutenant.
Larry Clark moved to approve the D.R.O.P. Enrollment Application submitted by John Robert Wolfe, Police
Lieutenant. City Manager Brenda S. Fischer seconded the motion. The motion passed unanimously.

Commencement of Executive Session
Mayor Weiers, recessed the Open Session and convened into Executive Session to review and discuss the
medical evaluations of nine (9) new Police personnel.
Reconvened into Open Session
The Board adjourned Executive Session and reconvened into Open Session to vote on the admission of nine
(9) Police personnel into the PSPRS with any pre-existing conditions noted on the medical evaluations.
Review and Approve Police Medical Evaluations for Pre-existing Conditions
Lt. John Wolfe made a motion to accept the applications of the following nine (9) Police personnel into the
PSPRS with any pre-existing conditions noted:
Adrian Anthony Aguirre Kristopher C. Larsen
Jonathan Daniel Armstrong Mitchell A. Mabry
Matthew John Bendokaitis Daniel Lee Perez
Cramer Lyle Doughty Alissandra D. Smith
Gennaro Sebastian Itri
Larry Clark seconded the motion. The Board voted and passed the motion unanimously.
New Business
Nick DiPiazza, Chief Deputy City Attorney, recommended that at the next joint meeting of the PSPRS Police
and Fire Boards, a review of the Open Meeting Law and Public Records Requests be done. He asked that it be
put on as an agenda item for the joint meeting.
Adjournment
Mayor Jerry Weiers asked for a motion to adjourn the meeting. Lt. John Wolfe made a motion to adjourn the
meeting. Larry Clark seconded the motion. The Local Police Board meeting adjourned at 9:25 a.m.
