Glendale Ave. Glendale, AZ MEMBERS PRESENT: Mayor Jerry Weiers, Chair Det. Brandon Keefer, Board Member Lt. John Wolfe, Board Member STAFF PRESENT: Jim Brown, Executive Director Human Resources and Board Secretary Nick DiPiazza, Chief Deputy City Attorney Michele Blanco, Police Personnel Management Marcia Zientek, Human Resources Support Staff MEMBERS ABSENT: City Manager Brenda S. Fischer, Board Member Larry Clark, Citizen Board Member Call to Order Mayor Jerry Weiers, Chair, called to order the Local Police Board meeting at 9:10 a.m. Review and Approval of the September 25,2013 Local Police Board Meeting Minutes The minutes of the September 25, 2013 Local Police Board meeting were reviewed. Lt. John Wolfe moved to accept the minutes as presented. Det. Brandon Keefer seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of Application for Normal Retirement—Willie Bishop,Jr. The Local Police Board reviewed Willie Bishop, Jr.'s Application for Normal Retirement. Willie Bishop, Jr., Police Officer, has completed 15.268 years of credited service. He is scheduled to retire on December 27, 2013. He has been a member of the Public Safety Personnel Retirement System (PSPRS) since September 21, 1998. The action requested of the Board is to approve the Normal Retirement Application for Willie Bishop, Jr., Police Officer. Det. Brandon Keefer made a motion to approve the Application for Normal Retirement submitted by Willie Bishop, Jr., Police Officer. Lt. John Wolfe seconded the motion. The Board voted and passed the motion unanimously. Page 1 of 3 PSPRS Local Police Board Meeting Minutes 10/23/13 Review and Consider Approval of Application for Separation Refund from the Public Safety Personnel Retirement System (PSPRS)—Brooke Contreras The Local Police Board reviewed the Application for Separation Refund from the PSPRS for Brooke Contreras, Police Officer. Brooke Contreras terminated her employment and certified that she has not been accepted for employment with any other Public Safety Personnel Retirement System (PSPRS) employer and has not previously received a refund of her accumulated contributions to the PSPRS. Human Resources certified that Brooke Contreras' last date of employment was September 27, 2013. The action requested of the Board is to approve the Application for a Separation Refund from PSPRS Retirement submitted by Brooke Contreras, Police Officer. Lt. John Wolfe made a motion to approve the Application for Separation Refund from the PSPRS submitted by Brooke Contreras, Police Officer. Det. Brandon Keefer seconded the motion. The Board voted and passed the motion unanimously. Acknowledgement of Death of Survivor—Earline Glidden The Local Police Board was asked to acknowledge the death of Earline Glidden, the surviving spouse of Virgil Glidden, retired Glendale Police Captain. Earline Glidden was approved for Survivor Benefits by the PSPRS Local Police Board on August 27, 2003. Mrs. Glidden passed away on June 26, 2013 and there were no remaining benefits to be distributed since there were no `eligible children' to receive any benefits and the pension that Mr. Glidden received overall was more than the contributions he paid into the plan. The action requested of the Board is to acknowledge the death of Mrs. Earline Glidden, surviving spouse of Virgil Glidden, retired Police Captain. Lt. John Wolfe acknowledged the death of Earline Glidden, surviving spouse of Virgil Glidden, retired Glendale Police Captain. Det. Brandon Keefer seconded the acknowledgement. Commencement of Executive Session Mayor Jerry Weiers, Chair, recessed the Open Session and convened into Executive Session to review and discuss the medical evaluation of one (1) new Police person. Reconvened in Open Session The Board adjourned Executive Session and reconvened into Open Session to vote on the admission of one (1) new Police person into the PSPRS with any pre-existing conditions noted on the medical evaluation. Review and Approve Police Medical Evaluation for Pre-existing Conditions A motion was made for the following one (1) new Police person to be voted on for admission into the PSPRS with any pre-existing conditions noted: Det. Brandon Keefer moved to accept Daniel Alfredo Gonzales into the PSPRS with any pre-existing conditions noted. Lt. John Wolfe seconded the motion. The Board voted & passed the motion unanimously. Page 2 of 3 PSPRS Local Police Board Meeting Minutes 10/23/13 New Business There was no new business. Adjournment Mayor Jerry Weiers, Chair, asked for a motion to adjourn the meeting. Lt.
