Call to Order
Assistant City Manager Horatio Skeete, Acting Chair, called to order the Local Fire Board meeting at
9:04 a.m.
Review and Approval of the May 22,2013 Local Fire Board Meeting Minutes
The minutes of the May 22, 2013 Local Fire Board meeting were reviewed. Mayor Jerry Weiers moved
to accept the minutes as presented. Capt. Mark Manor seconded the motion. The Board voted and passed
the motion unanimously.
Review and Approval of Application for Retirement from the Deferred Retirement Option Plan
("D.R.O.P.")—Lex Worthy
The Local Fire Board reviewed Lex Worthy's D.R.O.P. retirement application. Lex Worthy, Fire
Captain, has elected to retire from the D.R.O.P. and terminate his employment with the Glendale Fire
Department on June 26, 2013. On June 25, 2008,the Local Fire Board had approved Lex Worthy into the
D.R.O.P. At that time, he had completed 28.036 years of credited service. The action requested of the
Board is to approve the application for retirement from D.R.O.P. and Lump Sum distribution for Lex
Worthy,Fire Captain.
Mayor Jerry Weiers moved to approve the D.R.O.P. retirement application submitted by Lex Worthy, Fire
Captain. Capt. Mark Manor seconded the motion. The Board voted and passed the motion unanimously.

Review and A. s royal of A s s lication for Se s aration Refund from Public Safe Personnel
Retirement System ("PSPRS")—Todd Morrisson
The Local Fire Board reviewed the Application for Separation Refund from the PSPRS for Todd
Morrisson, Fire Fighter. Todd Morrisson has terminated his employment and certifies that he has not
been accepted for employment with any other Public Safety Personnel Retirement System (PSPRS)
employer and has not previously received a refund of his accumulated contributions to the PSPRS.
Human Resources certified that Todd Morrisson's last date of employment was December 30, 2002.
Todd Morrisson has elected to receive a Lump Sum Distribution.
Capt. Mark Manor made a motion to approve the Application for Separation Refund from PSPRS
submitted by Todd Morrisson, Fire Fighter. Mayor Jerry Weiers seconded the motion. The Board voted
and passed the motion unanimously.
New Business
There was no new business.
Adjournment
Assistant City Manager Horatio Skeete, Acting Chair, asked for a motion to adjourn the meeting. Mayor
Jerry Weiers moved to adjourn the PSPRS Fire Board meeting. Assistant City Manager Horatio Skeete
seconded the motion. The meeting was adjourned at 9:10 a.m.
