Glendale Ave. Glendale, AZ MEMBERS PRESENT: Larry Clark, Citizen Board Member and Acting Chair Det. Brandon Keefer, Board Member Lt. John Wolfe, Board Member STAFF PRESENT: Jim Brown, Interim HR Executive Director and Board Secretary Nick DiPiazza, Chief Deputy City Attorney Lorena Sanchez, Human Resources Support Staff MEMBERS ABSENT: Mayor Jerry Weiers, Board Member Acting City Manager Horatio Skeete, Board Member Call to Order Larry Clark, Acting Chair, called to order the Local Police Board meeting at 8:51 a.m. Review and Approval of the January 23,2013 Local Police Board Meeting Minutes The minutes of the January 23, 2013 Local Police Board meeting were reviewed. Lt. John Wolfe moved to accept the minutes as presented. Det. Brandon Keefer seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan ("D.R.O.P.")—Johnny Arevalo The Local Police Board reviewed Johnny Arevalo's D.R.O.P. Enrollment Application. Johnny Arevalo, Police Lieutenant, has 32.00 years of credited service. He elected to participate in the D.R.O.P. on February 9, 2013. He chose to participate in the D.R.O.P. for a period of sixty (60) months. On and after this date, the employee and employer contributions will cease to be made to the Public Safety Personnel Retirement System. He must terminate employment with the City of Glendale Police Department on or before February 9, 2018. He has been a member of the Glendale Police Department since March 11, 1980. The action requested of the Board is to approve the Application for Enrollment in D.R.O.P. Retirement submitted by Johnny Arevalo, Police Lieutenant. Lt. John Wolfe moved to approve the D.R.O.P. Enrollment Application submitted by Johnny Arevalo, Police Lieutenant. Det. Brandon Keefer seconded the motion. The motion passed unanimously. Page 1 of 2 PSPRS Local Police Board Meeting Minutes 3/6/13 Review and Approval of Application for Separation Refund from Public Safety Personnel Retirement System ("PSPRS")—Shane B. Leske The Local Police Board reviewed the Application for Separation Refund from the "PSPRS" for Shane B. Leske, Police Officer. Shane B. Leske certified that he has not been accepted for employment with any other Public Safety Personnel Retirement System (PSPRS) employer and has not previously received a refund of his accumulated contributions to the PSPRS. Human Resources certified that Shane B. Leske's last date of employment was January 25, 2013. Det. Brandon Keefer made a motion to approve the Application for Separation Refund from PSPRS submitted by Shane B. Leske, Police Officer. Lt. John Wolfe seconded the motion. The Board voted and passed the motion unanimously. New Business There was no new business. Adjournment Larry Clark, Acting Chair, asked for a motion to adjourn the meeting. Det.
