Glendale Ave. Glendale, AZ MEMBERS PRESENT: Mayor Elaine Scruggs, Present Telephonically Larry Clark, Citizen Board Member and Acting Chair Det. Brandon Keefer, Police Board Member STAFF PRESENT: Lupe Sierra, Deputy HR Director and Acting Board Secretary Nick DiPiazza, Chief Deputy City Attorney Adam Beghtol, Police Technical Services Manager Det. Michele Blanco, Police Officer Marcia Zientek, Human Resources Support Staff MEMBERS ABSENT: City Manager Ed Beasley, Board Member Lt. John Wolfe, Police Board Member Call to Order Larry Clark, Acting Chair, called to order the Local Police Board meeting at 9:00 a.m. Review and Approval of the June 22,2011 Local Police Board Meeting Minutes The minutes of the June 22, 2011 Local Police Board meeting were reviewed. Det. Brandon Keefer moved to accept the minutes as presented. Mayor Elaine Scruggs seconded the motion. The Board voted and passed the motion unanimously. Review and Approval of the Amended Application for Enrollment into the Deferred Retirement Option Plan ("D.R.O.P.")—Frank Balkcom, Sr. Frank Balkcom, Sr. elected to revise his participation date in the D.R.O.P. which was originally submitted and approved by the PSPRS Local Board at their May 25, 2011 meeting. Lt. Balkcom was originally scheduled to begin his participation in the D.R.O.P. on June 11, 2011. He revised his entry participation date into the D.R.O.P. and it will now be August 7, 2011. He is choosing to participate in the D.R.O.P. for a period of sixty (60) months as he originally applied. On or after this date, employee and employer contributions will cease to be made to the Public Safety Personnel Retirement System ("PSPRS"). He must terminate employment with the City of Glendale Police Department on or before August 7, 2016. The reason for the amendment is that it was determined by PSPRS that Lt. Balkcom, Sr. was missing four (4) pay periods of contributions towards the PSPRS which decreased his original credited service time. Lt. Balkcom, Sr. had several options available, and he chose to revise his date of entry into the D.R.O.P. in order make his credited service amount to 30.002 years. Lt. Balkcom, Sr. has been a member of the Glendale Police Department since April 23, 1982. Mayor Elaine Scruggs moved to approve the amended D.R.O.P. enrollment application submitted by Frank Balkcom, Sr., Police Lieutenant. Det. Brandon Keefer seconded the motion. The motion passed unanimously. Page 1 of 2 PSPRS Local Police Board Meeting Minutes 8/24/2011 Review and Approval of Application for Retirement from Deferred Retirement Option Plan ("D.R.O.P.")—Robert I. Hawkins The Local Police Board reviewed Robert I. Hawkins' D.R.O.P. retirement application. Robert I. Hawkins, Police Officer, has elected to retire from the D.R.O.P. and terminated his employment with the Glendale Police Department on July 28, 2011. On April 23, 2008, the Local Police Board had approved Robert Hawkins into the D.R.O.P. At that time, he had completed 22 years of credited service. Upon approval of his participation into the D.R.O.P., employee and employer contributions ceased to be made into the Public Safety Personnel Retirement System ("PSPRS"). He was required to terminate employment with the City of Glendale Police Department on or before April 23, 2013. Det. Brandon Keefer moved to approve the D.R.O.P. retirement application submitted by Robert I. Hawkins, Police Officer. Mayor Elaine Scruggs seconded the motion. The Board voted and passed the motion unanimously. Commencement of Executive Session Larry Clark recessed the Open Session and convened into Executive Session to review and discuss the medical evaluations of four (4) Police personnel. The Board adjourned Executive Session and reconvened into Open Session to vote on the admission of four (4) Police personnel into the PSPRS with the pre-existing conditions noted on the medical evaluations. Review and Approve Police Medical Evaluations for Pre-existing Conditions Det. Brandon Keefer made a motion to accept the applications of the following four (4) Police personnel into the PSPRS with any pre-existing conditions noted: Brian Matthew Draper Joel David Queenan William Harold Esh Allan John Seidl Mayor Elaine Scruggs seconded the motion. The Board voted and passed the motion unanimously. New Business There was no new business. Adjournment Larry Clark, Acting Chair, asked for a motion to adjourn the meeting. Det. Brandon Keefer made a motion to adjourn the meeting. Mayor Elaine Scruggs seconded the motion.
