Call to Order
Larry Clark, Acting Chair, called to order the Local Fire Board meeting at 8:13 a.m.
Review and Approval of the October 22, 2008 Local Fire Board Meeting Minutes
The Minutes of the October 22, 2008 Local Fire Board meeting were reviewed. Mark Manor moved to accept
the Minutes as presented. John Holland seconded the motion. The Board voted and passed the motion
unanimously.
Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan
("D.R.O.P.")—Dale R. Largent
The Local Fire Board reviewed Dale R. Largent's D.R.O.P. application. Dale R. Largent, Fire Captain,has 32
years of credited service. He had elected to participate in the Deferred Retirement Option Plan ("D.R.O.P.")
for a period of sixty (60) months effective November 30, 2008. On and after this date, the employee and
employer contributions would cease to be made to the Public Safety Personnel Retirement System
("P.S.P.R.S."). He must terminate employment with the City of Glendale Fire Department on or before
November 30, 2013. He has been a member of PSPRS since November 29, 1976.
John Holland made a motion to approve the D.R.O.P. application submitted by Dale R. Largent, Fire Captain.
Mark Manor seconded the motion. The Board voted and passed the motion unanimously.

Review and Approval of Application for Enrollment into the Deferred Retirement Option Plan
("D.R.O.P.")—Blair Kevin Petersen
The Local Fire Board reviewed Blair Kevin Petersen's D.R.O.P. application. Blair Kevin Petersen, Fire
Captain, has 26.130 years of credited service which includes 2.749 prior years of service. He had elected to
participate in the Deferred Retirement Option Plan ("D.R.O.P.") for a period of sixty (60) months effective
November 30, 2008. On and after this date, the employee and employer contributions would cease to be made
to the Public Safety Personnel Retirement System ("P.S.P.R.S."). He must terminate employment with the
City of Glendale Fire Department on or before November 30, 2013. He has been a member of PSPRS since
July 15, 1985.
Mark Manor made a motion to approve the D.R.O.P. application submitted by Blair Kevin Petersen, Fire
Captain. John Holland seconded the motion. The Board voted and passed the motion unanimously.
Review and Approval of Application for Normal Retirement—Thomas Shannon
The Local Fire Board reviewed Thomas Shannon's Application for Normal Retirement. Thomas Shannon,
Assistant Fire Chief, has completed 20.386 years of credited service. He retired from the Glendale Police
Department on November 14, 2008. He had been a member of the Public Safety Retirement System since
June 27, 1988.
John Holland made a motion to approve the Application for Normal Retirement submitted by Thomas
Shannon,Assistant Fire Chief. Mark Manor seconded the motion. The Board voted and passed the motion
unanimously.
Commencement of Executive Session
Larry Clark recessed the Open Session and convened in Executive Session to review and discuss the medical
evaluations for one (1) Fire person.
Before reviewing the one (1) medical evaluation, Alma Carmicle advised the Board that the Fire Person was
transferring from one Public Safety Personnel employer to another Public Safety Retirement System
employer.
The Board adjourned the Executive Session and reconvened into Open Session to vote on the admission of
one (1) Fire person into the PSPRS with the pre-existing conditions noted on the medical evaluation.
Review and Approve Fire Medical Evaluations for Pre-Existing Conditions
Mark Manor made a motion to accept the one (1) application for the following Fire person into the PSPRS
with the pre-existing conditions noted on the Phoenix Fire Department Health Center medical evaluation:

John Holland seconded the motion. The Board voted and passed the motion unanimously.

New Business
The Board was given a copy of the PSPRS Board Meeting calendar for 2009.
There was no other new business.
Adjournment
Larry Clark adjourned the Local Fire Board meeting at 8:23 a.m.
