1. CALL TO ORDER THE LOCAL POLICE BOARD
*The minutes of the January 24, 2018 PSPRS Police Board were approved with the following
.-amendment: City Manager Kevin Phelps called the meeting to order.
2. ROLL CALL
Members Present: City Manager Kevin Phelps
Board Member Justin Harris
Board Member Cody Carmichael
Members Absent: Mayor Jerry Weiers
Board Member Charles Erickson
Staff Present: Jim Brown, Nancy Mangone, Michele Blanco,
Vicki Moss, Charlotte Beadles, Alyssa Bethel
Other Attendees: Attorneys Gary Verburg and Carrie O'Brien
3. CITIZEN COMMENTS
No citizens were in attendance.
4. APPROVAL OF THE MINUTES
Public Safety Personnel Retirement System- Local Police Board Minutes of November 15, 2017.
Motioned by Board Member Justin Harris, seconded by Board Member Cody Carmichael
Vote: 3-0 Passed
5. NEW BUSINESS
a.Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.)—Aaron D.
Victor, Police Sergeant.Aaron D. Victor has 20.026 years of credited service. He elected to
participate in DROP on March 1, 2018 and must terminate employment with the City of
Glendale Police Department on or before March 31, 2023.
Motioned by Board Member Cody Carmichael, seconded by Board Member Justin Harris
Vote: 3-0 Passed
b.Application for Enrollment into the Deferred Retirement Option Plan (D.R.O.P.)—Jacqueline
Kay Cole, Police Sergeant. Jacqueline Kay Cole has 20.003 years of credited service. She
elected to participate in DROP on December 4, 2017 and must terminate employment with
the City of Glendale Police Department on or before December 31, 2022.
Motioned by Board Member Cody Carmichael, seconded by Board Member Justin Harris
Vote: 3-0 Passed
c. Application for Retirement from the Deferred Retirement Option Plan (D.R.O.P.)—Johnny
Arevalo, Police Lieutenant. Johnny Arevalo has elected to retire from the DROP and will
terminate his employment with the Glendale Police Department on February 28, 2018. On
February 27, 2013, the Local Police Board had approved an Application for Enrollment in
DROP and he had elected to participate on February 9. 2013. He was required to terminate
employment with the City of Glendale Police Department on or before February 28, 2018.
Motioned by Board Member Cody Carmichael, seconded by Board Member Justin Harris
Vote: 3-0 Passed
d.Application for Surviving Spouse Benefit—Sharon Alexander, spouse of Merrion Alexander,
Police Sergeant, deceased.
Motioned by Board Member Justin Harris, seconded by Board Member Cody Carmichael
Vote: 3-0 Passed
e.Application for Accidental Disability Benefits- David M. Kulaga, Police Lieutenant.An
application for Disability Retirement from the PSPRS has been submitted to the Human
Resources Department by David M. Kulaga. His full application along with supporting
documentation will be presented to the PSPRS Board at a later date.
6. EXECUTIVE SESSION
a. Medical Evaluations for One (1) New Police Personnel was reviewed by the Board - Byron
Carl Bullock.
b.Application for Accidental Disability Retirement from the Public Safety Personnel
Retirement System (PSPRS)- David M. Kulaga, Police Lieutenant. An application for
Disability Retirement from the PSPRS has been submitted to the Human Resources
Department by David M. Kulaga. His full application along with supporting documentation
will be presented to the PSPRS Board at a later date.
7. ADJOURN EXECUTIVE SESSION AND RECONVENE IN OPEN SESSION
8. CONSIDERATION AND POSSIBLE VOTE ON EXECUTIVE SESSION ITEMS
Admission of one (1) new Police personnel into the Public Safety Personnel Retirement System
(PSPRS). Officer Michele Blanco confirmed that the following new Police Personnel met the
Standards of the Glendale Police Department and the Arizona Peace Officer Standards and
Training Board:
Byron Carl Bullock
A motion was made by Board Member Cody Carmichael and seconded by Board Member Justin
Harris to accept the one (1)
new Police Personnel into the PSPRS with any pre-existing conditions noted.
Vote: 3-0
9. NEXT MEETING
The next regular meeting of the Public Safety Personnel Retirement System Local Police Board
will be held on February 28, 2018 at 9:30 a.m., in the 4th Floor Conference Room, located
at 5850 W. Glendale Ave., Glendale, Arizona, 85301.
10. ADJOURN THE LOCAL POLICE BOARD MEETING

were submitted and approved this 28th day of March, 2018.
